You name was not found in the user database.  This could mean a number of
things...

1.) There was a mistake in your data that caused it to be discarded by the
        system.
2.) You dropped carrier (or lost carrier) before the completion of the
        new user application.  (i.e. there was no data to insert into the
        database.

Please choose the "Apply" option and resubmit your application.
